Nidana Pañcaka in one clinical question

In a viva, the examiner may describe a patient who has irregular meals, night waking, abdominal distension, relief after passing flatus, and a later pattern of pain moving through the colon. Then comes the question: “Explain the Nidāna Pañcaka.” Many students know the five words but mix up the stage at which each word belongs.

Nidāna Pañcaka (the fivefold framework for understanding and diagnosing disease) is not merely a list to memorise. It is a sequence of questions: What caused the disease? What appeared before the disease became clear? What features define it now? What relieves or worsens it? Through what path did it develop? This article separates Hetu, Pūrvārūpa, Rūpa, Upaśaya and Samprāpti and anchors them in the classical texts.

Why Nidana Panchaka matters for your exams

What the classical texts say about Nidana Panchaka

Charaka: diagnosis through causes, features and development

The Nidānasthāna of the Charaka Saṃhitā is the principal source for studying disease diagnosis through the examination of causes, premonitory features, cardinal signs and pathogenesis. Its disease chapters repeatedly arrange the account of a disorder around nidāna (cause), pūrvārūpa (prodromal features), rūpa (manifest features) and samprāpti (pathogenesis). The Vimānasthāna of Charaka, especially its discussions of examination and disease knowledge, reinforces the need to understand the disease through its causative factors, doṣa involvement, location and progression.

In Charaka, the physician does not identify a disease from symptoms in isolation. The causative diet and behaviour, the doṣa disturbed, the site of lodging, the channels involved and the sequence of development all contribute to diagnosis. Cakrapāṇidatta’s commentary frequently clarifies that a disease description is not just a symptom catalogue: the factors are connected through a causal process.

Suśruta: diagnosis linked with examination and pathology

The Nidānasthāna of the Suśruta Saṃhitā presents the causes, premonitory features, signs, complications and development of important diseases. Suśruta’s wider diagnostic method, discussed across the Sūtrasthāna and Nidānasthāna, gives special importance to direct examination, the nature of the lesion and the anatomical site involved.

Suśruta’s presentation is particularly valuable when the disease has a visible or structural expression. The same general framework applies, but the examiner may expect you to connect Samprāpti with the affected dhātu (tissue), srotas (channel system) and site of disease. Ḍalhaṇa’s commentary is important for reading Suśruta’s technical distinctions, especially where a sign, site or pathological process is being differentiated.

Vāgbhaṭa: a compact and highly examinable formulation

The Nidānasthāna of the Aṣṭāṅga Hṛdaya gives concise accounts of disease causes, premonitory features, signs, progression and complications. Vāgbhaṭa’s presentation is especially useful for revision because the material is compressed into memorable diagnostic patterns. The Sūtrasthāna of the Aṣṭāṅga Hṛdaya also provides the conceptual foundation: doṣas become aggravated by causative factors, move through the body, lodge in susceptible sites and produce disease.

Aruṇadatta and Hemādri, the major commentators on Vāgbhaṭa, help explain how a disease’s visible expression follows from doṣa, dūṣya and location. When a question asks for the difference between a general sign and a disease-specific sign, or between a doṣa’s movement and its final localisation, the commentarial reading can sharpen the answer.

Mādhava Nidāna and the fivefold diagnostic method

The Mādhava Nidāna, a major text of the Laghu Trayī, is organised around disease diagnosis and is one of the most useful sources for the practical study of Nidāna Pañcaka. Its disease descriptions bring together nidāna, pūrvārūpa, rūpa, upaśaya and samprāpti, although the detail and emphasis may vary from disease to disease.

A useful examination point is that the five factors should not be treated as five unrelated definitions. Mādhava Nidāna shows their clinical purpose: the physician moves from cause to early change, from early change to clear manifestation, from testing relief or aggravation to understanding the disease process.

Are the five factors identical in every text?

The broad framework is stable, but the arrangement and amount of detail differ. Some texts explain Upaśaya explicitly; in other disease chapters, relief and aggravation are embedded in the description. Likewise, Samprāpti may be stated briefly in one chapter and elaborated through doṣa, dūṣya, srotas and mārga in another.

The safest answer is therefore not to claim that every author gives an identical standalone definition. State that the fivefold diagnostic framework is classically expressed through the disease descriptions of the major Saṃhitās and is especially systematised for study in Mādhava Nidāna.

The five factors: one question for each

1. Hetu: what caused the disease?

Hetu (the cause or etiological factor) is the factor that initiates or supports the disturbance leading to disease. In this context, Nidāna is often used in the sense of cause, although nidāna can also refer more broadly to the diagnostic account of a disease. Nimitta, āyatana and other causal terms may occur in related discussions, but for Nidāna Pañcaka the student should identify Hetu as the causative input.

Hetu may be understood through several familiar Ayurvedic categories. Āhāra (diet), vihāra (activity and behaviour), season, psychological factors, trauma, infection-like exposures described in the texts, and suppression of natural urges may all function as causes depending on the disease. A cause is not merely something that happened before the illness; it must have a meaningful relationship with doṣa aggravation, tissue disturbance or channel impairment.

Classically, causes may also be grouped according to their relation to the doṣas. Vāta-provoking factors include excessive exertion, fasting, irregular routine and exposure to cold or dryness; Pitta-provoking factors include excessive heat, sour or pungent intake and anger; Kapha-provoking factors include heaviness, excess nourishment, sleep and inactivity. These are not interchangeable labels: the relevant cause depends on the disease and its Samprāpti.

In Charaka’s disease discussions, Hetu is often followed by the doṣa-specific or disease-specific outcome. In the Nidānasthāna of Charaka, the student should read each cause together with the doṣa it aggravates and the disease it helps produce. That is the difference between listing a cause and explaining its role.

2. Pūrvārūpa: what appeared before full manifestation?

Pūrvārūpa (premonitory or prodromal feature) is the feature that appears before the disease has fully manifested. It signals that the disease process is underway, but it does not yet display the complete and distinctive picture of the disease.

Pūrvārūpa may be subtle, incomplete, vague or shared by more than one disease. In classical descriptions, it helps the physician recognise the approaching disease before the full set of Rūpa becomes evident. This is why the word “preliminary” is useful, but “preliminary symptom” should not be interpreted as an ordinary minor symptom; its defining quality is its stage in the disease process.

Students commonly confuse Pūrvārūpa with a mild Rūpa. The distinction is not simply severity. A feature is Pūrvārūpa because it occurs before full manifestation and remains insufficiently specific or incomplete; a feature becomes Rūpa when the disease has become manifest in its characteristic form.

Pūrvārūpa is sometimes discussed as sāmānya (general) and viśeṣa (specific), depending on whether the feature is common to a group of diseases or points more closely toward one disease. Some teaching traditions also distinguish clearly expressed and indistinct premonitory features. When writing an answer, define the stage first, then mention the classification used in your syllabus or source.

Example: In a disease whose complete picture includes fever with a recognisable pattern, early lassitude, heaviness or altered appetite may belong to the prodromal stage if they precede the established fever pattern. The exact examples must be taken from the disease chapter being asked, not invented as universal features.

3. Rūpa: what is manifest now?

Rūpa (the manifest sign or symptom) is the clearly expressed feature of disease. It is also called liṅga (indicator) or lakṣaṇa (characteristic sign) in related contexts, although the preferred term depends on the passage and the teaching tradition.

Rūpa gives the disease its recognisable clinical form. It can include subjective symptoms such as pain, thirst or nausea; objective signs such as colour, swelling or temperature change; functional disturbances such as altered digestion or movement; and disease-specific patterns such as the distribution, timing and quality of a complaint.

The best Rūpa answer is not a random list. Organise the features according to the classical description: general signs first if the text does so, then doṣa-specific signs, site-specific signs, complications or distinguishing features. If the question is about a disease, the exact order and terminology of the source should be preserved wherever possible.

Rūpa also has diagnostic value only in context. A symptom such as pain can occur in several disorders. Its quality, location, timing, associated features, causative history and response to Upaśaya determine its meaning. This is why the five parts of Nidāna Pañcaka work together rather than independently.

Exam discriminator: Rūpa is the disease as it is visibly or clinically expressed, not the cause and not the mechanism behind it.

4. Upaśaya: what relieves, and what does that reveal?

Upaśaya (relief, alleviation or a diagnostic test based on relief) is the factor that reduces the disease or its symptoms and thereby helps identify the disease process. Anupaśaya (non-relief or aggravation) is the opposite response. Upaśaya may arise from food, activity, medicine, environmental change or another intervention, but in an examination answer it should be linked to the underlying doṣa and disease mechanism.

The important point is that Upaśaya is not simply “whatever makes the patient feel better.” Its diagnostic value lies in the relationship between the intervention and the disease. If a measure that opposes the causative factor, aggravated doṣa or disease process produces relief, that response supports a particular understanding of the disorder.

These six forms are taught in the Ayurvedic discussion of Upaśaya, particularly in the diagnostic tradition associated with Charaka and later teaching texts. Exact wording and examples may vary by commentary and textbook. Therefore, do not force an example into a category unless the relation is clear.

An intervention may also be sāmānya (generally useful) or viśeṣa (specifically useful), depending on whether it gives broad or disease-specific relief. The central diagnostic method remains: observe what reduces or aggravates the condition, then interpret the response through doṣa, dūṣya and Samprāpti.

5. Samprāpti: how did the disease develop?

Samprāpti (the pathogenesis or unfolding of disease) explains the sequence through which aggravated doṣas interact with dūṣya (the tissues or structures affected), move through srotas (channels), reach a susceptible site and produce disease. It is the bridge between cause and manifestation.

The basic Ayurvedic model begins with Hetu. Repeated or intense causative factors disturb one or more doṣas. The disturbed doṣas interact with the relevant dūṣya, impair or alter the function of the involved srotas, and move through one of the mārga (pathways of disease). When they lodge in a vulnerable site, sthāna-saṃśraya (localisation in a site) occurs; the disease becomes more clearly recognisable and Rūpa emerges.

The process is not always linear in a simplistic sense. Strength of Hetu, strength of doṣa, weakness of dūṣya, season, constitution and the condition of digestion all influence the progression. Charaka’s discussions of disease development and Vāgbhaṭa’s concise pathogenesis accounts show why the same exposure may not produce the same disease in every person.

A standard account of the stages of disease development is the Ṣaṭkriyākāla (six stages of disease action):

  1. Caya — accumulation of a doṣa in its own site.
  2. Prakopa — aggravation or excitation of that accumulated doṣa.
  3. Prasara — spread of the aggravated doṣa beyond its original site.
  4. Sthāna-saṃśraya — localisation in a susceptible tissue or site.
  5. Vyakti — clear manifestation of the disease.
  6. Bheda — differentiation into subtypes, complications or advanced forms.

This order is examinable. Suśruta discusses the six stages in the Sūtrasthāna, and the framework is central to Ayurvedic understanding of how a latent disturbance becomes a manifest disease. The fifth stage, Vyakti, corresponds most closely to the clearly expressed disease form, while Pūrvārūpa is associated with the stage immediately before full manifestation, especially the stage of localisation and early expression.

Another useful way to write Samprāpti is through its components: doṣa, dūṣya, srotas, mārga, agni (digestive and metabolic fire), āma (improperly processed or accumulated metabolic material, in contexts where the concept applies), site and the nature of the pathological interaction. Do not mechanically insert every component into every disease. Use only what the classical disease account supports.

How the five factors fit together

Consider a classical-style example without turning it into a treatment instruction. Suppose repeated dry, irregular eating and excessive exertion aggravate Vāta. That history belongs to Hetu. Early variable appetite, mild distension or a vague sense of discomfort before the established pattern belongs to Pūrvārūpa, if the disease chapter identifies such a stage.

Once the disorder is fully expressed through characteristic pain, distension, altered bowel movement and a recognisable pattern, these are Rūpa. If a measure that appropriately opposes the relevant Vāta qualities brings relief, that response is Upaśaya. The explanation of how aggravated Vāta interacts with the relevant site and channels to produce the pattern is Samprāpti.

Notice that the same fact can be interpreted differently depending on the question. Irregular eating is a cause; pain after that cause is a manifestation; relief after a suitable opposing measure is Upaśaya. The path connecting them is Samprāpti.

Nidana Panchaka versus the stages of disease

Nidāna Pañcaka and Ṣaṭkriyākāla are related but not identical. Nidāna Pañcaka is a diagnostic framework: it tells the physician what kinds of evidence to collect. Ṣaṭkriyākāla is a temporal model of disease progression: it explains how doṣa disturbance moves from accumulation to advanced differentiation.

A full answer can connect them, but should not substitute one for the other. Saying “the six stages are the five diagnostic factors” is incorrect.

Applied understanding: reading a case without giving treatment advice

In applied study, Nidāna Pañcaka helps you read a case as a pattern rather than as an isolated symptom. First identify the relevant exposures and habits. Next ask whether there was an early, incomplete phase. Then identify the fully expressed features and their sequence. Finally, use the response to appropriate opposing factors as supporting evidence and explain the underlying doṣa–dūṣya process.

This is an academic method of case interpretation, not medical advice. A modern clinician may compare parts of this process with history-taking, symptom analysis, response-based assessment and pathophysiological reasoning, but that is a modern correlation, not an assertion that the classical categories are identical to modern diagnostic categories.

By way of modern correlation, Hetu resembles an etiological history, Pūrvārūpa resembles prodromal change, Rūpa resembles the presenting clinical picture, Upaśaya resembles a response-based diagnostic clue, and Samprāpti resembles a model of disease development. The correspondence is helpful for learning, but the Ayurvedic concepts remain grounded in doṣa, dūṣya, srotas, guṇa and localisation.
