Charaka’s descriptions make clear that diagnosis is not restricted to the final symptom list: the physician must understand causative factors, the earliest signs, the developed disease and the process by which doṣas and dūṣyas become involved.

The Vimānasthāna of Charaka also supplies the wider diagnostic method. Its discussions of examination, disease knowledge, proper inference and the assessment of a patient show that a disease is understood through causes, signs, progression, strength and context. Cakrapāṇidatta’s commentary is especially useful here because he repeatedly clarifies whether a statement concerns the cause, the manifestation or the mechanism of disease.

In Charaka, Nidana often includes both direct causes and the circumstances that disturb doṣas. The Nidānasthāna also shows that the same cause may produce different diseases depending on doṣa predominance, tissue involvement, strength, season, age and channel vulnerability. Therefore, Nidana is not a mechanical one-cause-to-one-disease formula.

Suśruta’s contribution

The Nidānasthāna of Suśruta Saṃhitā presents the causes, premonitory signs, signs and symptoms, varieties and pathogenesis of important diseases. Suśruta’s surgical and anatomical orientation gives particular importance to localisation, structural involvement, progression and the distinction between superficial and deeper disease.

The Śārīrasthāna of Suśruta is relevant because it develops the bodily structures, channels and physiological foundations through which disease becomes manifest. Ḍalhaṇa, commenting on Suśruta, is valuable when the wording permits more than one reading. Vāgbhaṭa’s contribution

The Nidānasthāna of Aṣṭāṅga Hṛdaya compresses a large amount of diagnostic knowledge into a highly examinable format. Vāgbhaṭa systematically presents causes, early signs, established signs and samprapti in disease chapters, making the five-part framework easy to apply.

The Sūtrasthāna of Aṣṭāṅga Hṛdaya provides the foundational language of doṣa, dhātu, mala, Agni, Srotas and disease development. Aruṇadatta and Hemādri, the major commentators on Aṣṭāṅga Hṛdaya, are important for interpreting technical terms and resolving apparent differences in classification. Nidana Panchaka: understand the five components

1. Nidana: the cause and provoking factor

Nidana means the causative or disease-producing factor; in many contexts it also carries the sense of diagnostic basis or knowledge of disease. In a disease chapter, Nidana usually refers to the dietary, behavioural, environmental, psychological or other factors that provoke doṣas and initiate the disease process.

For example, excessive intake of heavy, sweet and unctuous food may support Kapha and Medas disturbance in a susceptible person. The statement is not complete merely because a food is labelled as a cause. Its effect depends on quantity, frequency, Agni, season, activity, constitution and the state of the channels.

Classically, causes may be understood by their relationship to doṣas: Vāta-provoking, Pitta-provoking or Kapha-provoking factors, as well as causes that disturb more than one doṣa. They may also be grouped by source, such as diet, regimen, season, mental factors, trauma, infection-like exposures or suppression of natural urges, depending on the disease under discussion.

A useful distinction is between general and specific causes. A general cause may increase a doṣa broadly, whereas a specific Nidana directs that disturbance toward a particular tissue, channel or organ. The Nidānasthāna of Charaka and the Nidānasthāna of Suśruta repeatedly show this movement from cause to doṣa disturbance and then to disease expression.

Nidana is not the same as a symptom. If a student writes fever, thirst and body ache under Nidana for Jvara, the categories have been mixed. Those are Rupa or, depending on their timing and context, Purvarupa; Nidana would include the provoking factors described in the classical Jvara chapter.

2. Purvarupa: the premonitory sign

Purvarupa means the feature that appears before the complete disease picture is established. It is an early, incomplete or characteristic indication that the disease is developing, but it may not yet permit confident identification of the fully developed disorder.

The Purvarupa may be vague or specific. A vague prodrome can indicate that illness is approaching without identifying its exact form. A specific prodrome is more closely connected with the disease and can make the diagnosis easier, especially when read together with Nidana and the patient’s context.

The essential temporal distinction is simple: Purvarupa belongs to the stage before complete manifestation; Rupa belongs to the stage in which the disease is manifest. The same feature may be described differently in different diseases or stages, so the student should not classify a symptom by its name alone. Classify it by its place in the disease sequence.

The disease chapters of Charaka, Suśruta and Vāgbhaṭa often list Purvarupa immediately before Rupa. That sequence itself is examinable. When reproducing a classical disease description, preserve the order rather than rearranging features into a modern symptom list.

3. Rupa: the established sign and symptom

Rupa means the manifest form or recognisable clinical expression of disease. It includes the signs and symptoms that become apparent after the pathological process has developed sufficiently for the disorder to be identified.

Rupa may be divided in teaching practice into general and specific features. General features support recognition of the disease family, while specific features help distinguish one disease, doṣa predominance or subtype from another. The disease chapters of the mūla texts commonly proceed from general description to varieties, complications and prognosis.

Rupa is not simply whatever the patient reports. Classical diagnosis integrates darśana, sparśana and praśna—observation, examination by touch and questioning—along with inference from the overall pattern. The Vimānasthāna of Charaka and the Sūtrasthāna of the major texts support this broader method of clinical assessment.

A high-quality answer should connect Rupa to doṣa predominance. A dry, rough, mobile and painful presentation suggests a different doṣic pattern from one marked by heat, redness and burning, or from one marked by heaviness, pallor and excessive mucous quality. These are not isolated adjectives; they are clinical expressions of guṇa, the qualities through which doṣas operate.

4. Upashaya: relief or aggravation as a diagnostic test

Upashaya means a measure, substance, diet, behaviour or circumstance that produces relief or sometimes aggravation, thereby helping the physician understand the disease. It is not merely the final treatment prescribed after diagnosis. Its diagnostic value lies in what the response reveals.

An Upashaya may act by pacifying the responsible doṣa, opposing the involved guṇa, improving Agni, clearing a channel or addressing a particular mechanism. Anupashaya is the non-relieving or aggravating response. Both responses may be informative when interpreted carefully.

For example, if a pattern suspected to arise from excessive coldness and heaviness improves with measures possessing warmth and lightness, that response supports the proposed doṣa-guṇa interpretation. It does not prove the diagnosis in isolation. Upashaya must be read alongside Nidana, Purvarupa, Rupa and Samprapti.

This is why Upashaya is sometimes described as a form of practical inference. The physician tests a hypothesis about the disease process and observes the response. The approach is classical reasoning, not a licence for unsupervised experimentation. In an academic answer, keep the discussion diagnostic and do not turn it into a real-world treatment recommendation.

5. Samprapti: the pathogenesis and disease development

Samprapti means the process by which disease develops. It explains the movement and interaction of aggravated doṣas with dūṣyas, the site of lodging, the affected Srotas, the role of Agni and the sequence through which signs and symptoms arise.

The classical pathogenesis is not simply a modern anatomical lesion description. It is a functional and relational account: which doṣa is disturbed, which tissue or waste product is affected, where the interaction occurs, what obstructs or alters movement, and why the particular clinical picture results.

The six stages of disease development known as Kriyākāla are often used to understand Samprapti: Sañcaya, Prakopa, Prasara, Sthānasamśraya, Vyakti and Bheda. In English, these may be rendered as accumulation, aggravation, spread, localisation, manifestation and differentiation or complication. The exact explanatory emphasis varies across textual and commentarial traditions, but the sequence is an essential examination framework.

The classical order is examinable: Sañcaya → Prakopa → Prasara → Sthānasamśraya → Vyakti → Bheda. Do not write the six stages in a random order. Suśruta’s Sūtrasthāna is especially important for the Kriyākāla teaching, while the disease Nidānasthāna of the Saṃhitās applies the logic to specific disorders.

Samprapti can also be described through the dominant doṣa, affected dūṣya, site, channel, type of obstruction or abnormal movement, and strength of the disease. A complete disease answer therefore does more than write Vāta-Pitta-Kapha. It identifies the relevant components and connects them causally.

How the five parts work as one diagnostic chain

The five components are not five unrelated headings. They form a reasoning chain:

  1. Nidana disturbs one or more doṣas.
  2. The disturbed doṣas interact with Agni, dūṣyas and Srotas.
  3. During early development, Purvarupa appears.
  4. Once the disease is established, Rupa becomes evident.
  5. The response to Upashaya or Anupashaya tests the proposed interpretation.
  6. Samprapti ties all these observations together into an explanation of the disease process.

The list above is a teaching sequence, not a rigid claim that every patient is examined in exactly that order. In a case, the physician may recognise Rupa first and then infer Nidana and Samprapti. In a written answer, however, the conventional order Nidana, Purvarupa, Rupa, Upashaya and Samprapti should be preserved unless the question asks otherwise.

A worked conceptual example: a doṣa-based pattern

Suppose a disease description contains repeated exposure to dry, light, irregular and excessive activity; early variable discomfort and anxiety; later pain, distension and irregular movement; improvement with opposing qualities; and a pathogenesis in which Vāta becomes aggravated and occupies a susceptible channel.

The repeated exposure belongs under Nidana. The early variable discomfort belongs under Purvarupa if it precedes the established picture. Pain, distension and altered movement belong under Rupa once the disorder is manifest. Improvement with an opposing measure is Upashaya. The account of Vāta aggravation, channel entry, localisation and expression is Samprapti.

This example is deliberately generic. In an examination, do not force a disease name from one isolated sign. First identify the five categories, then correlate them with the complete classical description of the named disorder.

Samprapti and its components: what a complete answer adds

Students often reduce Samprapti to a one-line doṣa statement. A stronger answer identifies Samprapti Ghaṭaka, the components of pathogenesis. The exact list may vary according to the teaching tradition and disease, but the following headings are commonly useful:

Samprapti versus modern pathophysiology

In classical doctrine, Samprapti explains disease through doṣa, dūṣya, Agni, Srotas, site and progression. By way of modern correlation, a student may compare this with a pathophysiological sequence involving risk factors, biological mechanisms, tissue changes and clinical manifestations.

The comparison can aid understanding, but it is not an equation. Doṣa is not identical to a single modern biochemical substance, Srotas is not always identical to one anatomical tube, and Agni should not be reduced automatically to gastric acid or one enzyme. In an examination, present the classical explanation first and label any modern comparison clearly as correlation.

The diagnostic value of Upashaya

Upashaya deserves special attention because it is the component most often memorised without being understood. Its logic is: if a suspected mechanism is correct, an appropriately opposing or supporting measure should produce a meaningful response.

This can be represented as a reasoning model:

  • A suspected doṣa or guṇa is identified from Nidana and Rupa.
  • A measure with an opposite or corrective property is considered.
  • Relief supports the proposed interpretation; aggravation may also reveal the relevant mechanism.
  • The response is weighed against the rest of the diagnostic evidence.

The response is not always straightforward. A temporary improvement may conceal an unresolved cause, and a measure may act through more than one mechanism. Therefore Upashaya is supportive evidence, not an independent replacement for complete examination.

The distinction between Upashaya and Anupashaya also helps answer viva questions. Upashaya is the relieving or diagnostically favourable response; Anupashaya refers to a response that does not relieve or that aggravates. Both are meaningful only in relation to the disease hypothesis.

Applied understanding: using Nidana Panchaka in a case
